Ramesh arrived without an appointment on a Thursday afternoon, holding his right cheek. He was 45, a government officer from Govindpura, and he had been managing a dull ache for 3 weeks before the pain escalated overnight.
"I was going to come eventually," he said, lowering himself into the examination chair carefully. "I just kept telling myself it would settle."
The clinical picture was straightforward — a periapical abscess on the upper right second premolar. The tooth had a failing old amalgam filling, secondary decay underneath, and the infection had tracked to the apex of the root. The adjacent tissue was slightly swollen. He had been managing it with over-the-counter painkillers for two weeks.
I explained the situation directly. "This tooth needs root canal treatment today to drain the infection and clean the canals. Once that is done, we will put a temporary filling and review in 2 weeks. If the bone heals well on the X-ray, we cap it with a crown and it serves you for another twenty years."
He was not expecting to have treatment the same day. "I thought you would just give me antibiotics and send me away."
"Antibiotics manage the infection systemically — they do not clean the source. The bacteria are inside the tooth. We need to go in."
The root canal took 55 minutes under local anaesthesia. Three canals, moderate curvature on the palatal root, all cleaned and shaped to the apex. A calcium hydroxide dressing was placed and the tooth temporised. Ramesh left with an antibiotic prescription for 5 days and instructions to eat soft food on the left side.
Two weeks later, the swelling had resolved and the probing was clean. The 2-week X-ray showed early periapical clearing. The tooth was obturated at that appointment and a zirconia crown was cemented 10 days later after the lab work was completed.
What began as an emergency turned into a complete treatment journey.
